Stoneybrook Assisted Living apartments provides an alternative safe and secure environment for individuals requiring minimal assistance but wishing to live as independently as possible.  In order to gain an accurate understanding of the individual’s needs, an assessment is required prior to admittance so the appropriate level of services can be determined.  Once in residence, an assessment will be completed regularly to insure adequate level of services are provided. 

These apartments are secure yet private permitting both single and double occupancy.  The assisted living services are delivered in four levels.  These levels are defined in terms of the services and care required and the daily rates correspond accordingly.  The living arrangements are designed to for privacy and maximum independence.  Each apartment features a kitchenette, private bathroom, emergency call system and a private entrance.  In addition heating and cooling system give residents complete control.  

Basic services include:

·         Three home-style meals per day served in a friendly, home-like dining room

·         All utilities except telephone and cable TV

·         Emergency call systems in each unit for 24-hour per day response

·         Activity and social programs

·         Scheduled transportation in and around the greater Manhattan community

·         Daily well being checks

·         Monthly health and medication review

·         Weekly laundering of bed linens

·         All interior and exterior maintenance services

·         Personal shopping service

·         Trained staff on duty 24 hours/7days a week

The Assisted Living agreement provides for priority access to the health care facility when the individual requires such care provided the accommodation is available.  No additional entrance fee is charged and the prevailing daily healthcare rate will be assessed.
